Q: Is Southeastern Virginia Health System a free clinic?
A: No, SEVHS is not a free clinic. SEVHS is a community health center that offers primary care to everybody. SEVHS accepts patients with or without health insurance. Payments for services are based upon a sliding fee scale.
Q: What is a sliding fee scale?
A: It is a discount. A sliding fee scale means that your payment is based upon your household income and family size. To apply for the sliding fee scale, you are required to show proof of household income by providing two current pay stubs or a W-2 form. If you have no household income, you must attest to that fact in writing.
Q: Do I have to live in the same city as Southeastern Virginia Health System office locations to be seen?
A: No, there are no current residential restrictions. SEVHS sees patients from many communities throughout Virginia.
Q: I have insurance, does Southeastern Virginia Health System accept insurance?
A: Yes, SEVHS accepts most major medical and dental insurance plans, including Medicare and Medicaid. We also offer a sliding fee scale based upon your household income and family size.
Q: I do not have insurance. Can I still be seen?
A: Yes, SEVHS accepts uninsured as well as underinsured patients. Your fees will be calculated according to a sliding fee scale, which is based upon your household income and family size.
Q: What should I bring with me to my appointment?
A: You should bring a government issued picture identification card that is unexpired, your insurance card if insured, current medications or a list of any medications that you are currently taking and, of course, your nominal fee. To apply for the sliding fee scale, you must bring your last 2 pay stubs or other proof of household income. Plan on arriving for your appointment at least 30 minutes early to complete the patient registration process.
Q: How can I get a copy of my medical record?
A: We need to receive a medical records release form signed by you before your records can be released. You may also be able to pick up your records in person, however, please allow sufficient time for your records to be retrieved and printed as same day requests cannot always be fulfilled. You should have your photo id ready when picking up your records in person. You may also incur a small fee when you opt to pick up your records in person.
Q: Do you perform tests for Sexually Transmitted Diseases and HIV/Aids?
A: Yes, we can perform these tests, however you must make an appointment to see a physician who will then be able to order these tests. Also, you may wish to consider contacting your local Health Department who can also perform these tests often at no charge.
Q: What exactly is the Joint Commission?
A: SEVHS is accredited by the Joint Commission. The Joint Commission is an independent, not-for-profit, national body that oversees the safety and quality of health care and other services provided to accredited organizations. Information about the accredited organizations may be provided directly to The Joint Commission at 1-800-994-6610.
